The gel pad sits on the inside of the sleeve, right over the joint that sticks out, and it spreads the pressure from the shoe instead of letting it land on one spot. The sleeve pulls on over the forefoot like a short sock with an opening for the big toe, and it grips without chafing. You wear it through the day in ordinary shoes, and it is thin enough not to show. Wash it by hand in lukewarm water and let it air dry.
